Acknowledgement of Yarmouth Little League Safety Guidelines 2020 Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and show no symptoms and, therefore, spread the disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death. As of June 2020, the State of Maine has reported more than 2,600 cases and 100 deaths.

Yarmouth Little League has implemented safety guidelines for the 2020 season to reduce the risk of becoming exposed to, contracting, or spreading COVID-19. The safety guidelines have been approved by the Town of Yarmouth. Your signature below is an acknowledgement that you have read, understand, and agree to follow the safety guidelines. Your signature below is further acknowledgement that Yarmouth Little League and/or Town officials have the right to remove you and/or your player from the fields complex for failure to follow the safety guidelines. Your signature below is further acknowledgement that Yarmouth Little League reserves the right in its sole discretion to suspend and/or cancel the season if circumstances warrant.

I hereby choose to accept the risk of contracting COVID-19 for myself and my child/player in order to participate in YLL activities and/or visit the fields complex. WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Yarmouth Little League and its board members, officers, coaches, volunteers, agents, and all others associated with the operation of the YLL 2020 season in connection with exposure, infection, and/or spread of COVID-19 related to participation and/or visits to the fields complex during the YLL 2020 season. CHOICE OF LAW: I understand and agree that the law of the State of Maine will apply to this contract.
