COVID-19 Waiver

Yarmouth Little League has implemented safety guidelines for the 2021 season to reduce the risk of becoming exposed to, contracting, or spreading COVID-19. The safety guidelines have been accepted by the Town of Yarmouth. Your signature below is an acknowledgement that you have read, understand, and agree to follow the safety guidelines. Your signature below is further acknowledgement that Yarmouth Little League and/or Town officials have the right to remove you and/or your player from the fields complex for failure to follow the safety guidelines. Your signature below is further acknowledgement that Yarmouth Little League reserves the right in its sole discretion to suspend and/or cancel the season if circumstances warrant.

Yarmouth Little League cannot prevent you or your child/player from becoming exposed to, contracting, or spreading COVID-19 while participating in YLL activities or visiting the fields complex. It is not possible to prevent against the presence of the disease. Therefore, if you choose to register and/or participate in YLL activities you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and my child/player in order to participate in YLL activities and/or visit the fields complex. Participating in YLL activities this season is of such value to me and my child/player that I accept the risk of being exposed to, contracting, and/or spreading COVID-19.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Yarmouth Little League and its board members, officers, coaches, volunteers, agents, and all others associated with the operation of the YLL 2021 season in connection with exposure, infection, and/or spread of COVID-19 related to participation and/or visits to the fields complex during the YLL 2021 season. I understand that this waiver means I give up my right to bring any claims (known or unknown) including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

CHOICE OF LAW: I understand and agree that the law of the State of Maine will apply to this acknowledgement/waiver.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS ACKNOWLEDGEMENT/RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS AND CHILD/PLAYER’S RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE:
